THANK YOU FOR YOUR INTEREST IN SACRED HEART SCHOOL!
This
page will provide you with the documents and information you need to
make application to our school for the 2009-2010 school year.

Stop by between 8:00 a.m. - 4:00
p.m. and drop off the completed appropriate forms at the front
desk, or mail them to us at P.O. Box 3672 Glyndon, MD 21071.

If you are new to our school,
please enclose a non-refundable
$50 New Family Application Fee.
Please make all checks payable to Sacred Heart School.

Please enclose a copy of your
child’s
birth certificate
as well as a copy of his/her
baptismal certificate
(if available)

If your child is currently
enrolled in another school, sign the
Transcript Release Form
and give it to your child’s current school so they have your
permission to forward your child’s academic record to us upon
acceptance.

If your child is currently
enrolled in another school, give the
Referral Form
to your child’s current teacher and ask them to complete it ASAP
and mail it or FAX it to us at (410) 833-0914.

If your child is old enough to be
receiving report cards, please send us a copy of
last year’s end-of-year report card
as well as
this year’s most recent report card.

If your child has participated in
any
standardized testing
(IOWA, Stanford 10, MSA, Terra Nova, etc) please send us copies
of those scores.

Please complete the
Health Records Release Form
and the
Tuition Option
Form to us. If you elect to use
our tuition management program, SMART applications must be
picked up at our school, or if requested, we will mail them to
you. They are not available electronically.
